Health and wellbeing for care leavers

Our Leaving Care Team and your Personal Advisor can support you to stay healthy and look after your physical and mental health and well-being.
Your Pathway Plan will include any health needs that you may have, as well as your NHS number, GP and Dentist information.
It is important that you take care of yourself and speak to your Personal Advisor about any health concerns.
Below, you can find local organisations that provide support with Drug and Alcohol Use, Sexual Health and Mental and Emotional Health.
NHS App
The NHS App will be your digital Health passport. You can use it to:
- view your health history
- get online health advice
- make GP appointments online (with participating GPs)
Compass – Harrow Young People’s Substance Misuse Service
A free confidential service for 5 to 24 year olds who need support around their own or another person’s (loved one) drug or alcohol substance use.
Compass Elevation is co-located with other providers of children and young people’s services. These services include support around domestic violence, sexual health (including free condoms and HIV testing) and mental health, as well as careers and employment opportunities.
Our caring and friendly team are available at a time that suits you and at a place where you feel comfortable.

Sexual Health
You can get free condoms at any pharmacies that display the C-Card.
Caryl Thomas Clinic
The Caryl Thomas Clinic is a sexual health and contraceptive service in Wealdstone.

Sexual Health London
An online sexual health service is available to residents of Brent, Ealing and Harrow through Sexual Health London.
They offer a straightforward and convenient test for sexually transmitted infections, without no need to visit a clinic. SHL’s testing service is for people who have mild or no symptoms.
You can also request free regular and emergency contraception (effective up to 5 days after unprotected sex) by completing a simple, secure online consultation.
Mental and Emotional Health
If you are in crisis, please call your Personal Advisor or emergency services 999 straight away for immediate help. If you’ve hurt yourself, it’s important you go to Accident and Emergency quickly so you can receive medical treatment.

Kooth is commissioned by the NHS, Local Authorities, charities and businesses to provide anonymous and personalised mental health support for Children and Young People.
They provide end to end support whatever the need. Kooth.com is an anonymous site where you can feel safe and confident to explore concerns and seek professional support. Access to Kooth is immediate. There is no need for a referral, no waiting lists and the service is available 24/7.
HeadsUp
HeadsUp offers a selection of support including one-to-one and group support sessions, depending on the needs of the young person. They work together with other organisations that specialise in:
- LGBT+ together with the Mosaic Trust
- Refugees and asylum seekers together with Paiwand
- ADHD and autistic young people together with the Centre for ADHD & Autism Support
- Support for self-harm together with The Wish Centre
